Lifestyle health coach (English- and/or Spanish/French)

Job Description

Lifestyle health coaches provide an individualized health improvement program to participants in brief sessions, typically 15 minutes, over the telephone. Coaches are trained to deliver a standardized, evidence-based program that addresses nutrition, weight management, physical activity and exercise, stress management, tobacco cessation, risk factor reduction (blood pressure, cholesterol, triglycerides), diabetes self-management, sleep/fatigue management and preventive exams and immunizations. The lifestyle management program is supported by a computerized tracking and management system and comprehensive educational materials.

Minimum Qualifications and Requirements

  • Bachelor's degree in exercise science, nutrition, nursing or other health-related field
  • Two years of relevant experience in a health-related field
  • Fit and healthy role model (non-smoker, healthy weight, etc.)
  • A caring and compassionate attitude
  • Excellent computer skills
  • Excellent communication skills
  • Successful completion of pre-training and on-site training activities
  • Own a printer, scanner and personal computer with high-speed Internet access (for coaches working remotely)

Training

Training focuses on the use of the computer management system, education content and coaching techniques for delivery of the coaching program. The computer management system and all training activities are provided in English. All participant materials (assessments, reports, educational materials and tools) are available in Spanish.

Pre-training is completed as self-study. Training is provided through web-based meetings. At the conclusion of the training, coaches must demonstrate mastery on practical and written exams. All coaches attend a one-hour weekly staff meeting on Friday mornings.

Position

INTERVENT approved, independent lifestyle health coach

Job Description

Lifestyle health coaches provide an individualized health improvement program to participants in brief sessions, typically 15 minutes, over the telephone. Coaches are trained to deliver a standardized, evidence-based program that addresses nutrition, weight management, physical activity and exercise, stress management, tobacco cessation, risk factor reduction (blood pressure, cholesterol, triglycerides), diabetes self-management, sleep/fatigue management and preventive exams and immunizations. The lifestyle management program is supported by a computerized tracking and management system and comprehensive educational materials.

Once training is completed, other requirements are met and a fee is paid, a formal license and services agreement with INTERVENT will permit the independent lifestyle health coach be in business for themselves and deliver an INTERVENT approved and maintained version of the program to adults living in the coach's defined service area. The coach may also provide coaching (in committed half-day schedule slots, paid by INTERVENT) to participants assigned by INTERVENT.

License and training fees apply.

Customer Care Representative

Duties and Responsibilities:

  • Conducts enrollment, scheduling, tracking, inventory management, fulfillment and follow-up as required for the delivery of comprehensive lifestyle management and risk reduction programs
  • Works to ensure optimum participant satisfaction and encourages program compliance in a professional manner
  • Promptly answers, screens and processes health risk assessment calls and inquiries with strict adherence to confidentiality agreements, policies and procedures
  • Provides an excellent first source for information about the INTERVENT program, policies and procedures
  • Manages coach availability; schedules appointments for participants and reschedules appointments as needed
  • Manages inquiry maintenance and converts inquiries to participants by understanding risk stratification and triage process
  • Assists with communications and incentives
  • Tracks and logs calls using data tracker/protector and processor
  • Manages coach tracking statistics for weekly reporting at team meeting

Qualifications and Requirements:

  • High school diploma (or the equivalent); college coursework/degree preferred
  • One year of experience in a customer service environment
  • Excellent typing and computer skills that include experience in a Windows Operating System, email and data entry
  • At least six months of phone experience in a previous job or volunteer position
  • Pleasing telephone voice
  • Fluency in Spanish or French is desirable
  • Excellent interpersonal and organizational skills
  • Intuitive mindset with effective problem solving skills
  • High energy level when answering incoming calls
