This unprecedented partnership between ACC and INTERVENT culminated in the receipt of the Best Poster Award for the following research presented at the ACC’s 66th Annual Scientific Session & Expo, held in Washington, DC in 2017: Evaluation of the CardioSmart Population Health Management Initiative: Changes in Health Risks and Participant Satisfaction. Medically-supervised weight management program with a comprehensive focus

Solution

We work with a team of physicians and clinicians in Toronto, Canada. Like in the U.S., there’s a high prevalence of overweight and obesity in Canada. INTERVENT is working with this team to offer an innovative, medically-supervised weight management program. Patients, especially those with diabetes and prediabetes, are achieving significant results in as little as eight weeks. Some patients use this program as an alternative to bariatric surgery. Others, who are facing bariatric surgery, use the program to prepare for and recover from the surgery.

A cardiologist practicing in Dallas, TX has referred hundreds of his patients to the INTERVENT telephonic health coaching program. cardiac rehabilitation program

This regional healthcare system had experienced declining enrollment in its traditional cardiac rehabilitation program over the past several years. It was clear that cardiac patients needed options for secondary prevention beyond the traditional onsite program. INTERVENT licensed its intellectual property to the healthcare system and trained the staff to use its online platform and educational materials to deliver an enhanced version of cardiac rehab that includes options for home-based telephonic lifestyle health coaching. As a result of the training, the cardiac rehabilitation staff became proficient in motivational interviewing and other proven coaching models. Participation in cardiac rehabilitation is now on the rise.
